Concierge primary care is a membership-based approach designed to create a closer, more responsive relationship between you and your doctor.
The model in plain English
Patients pay a membership fee that allows a physician to care for fewer people than in a typical high-volume practice. In return, members receive enhanced access, longer visits, closer follow-up, greater continuity, and more time devoted to prevention and health planning.
At APG, your membership relationship is directly with Christopher Green, MD. You continue to use Medicare or health insurance for covered medical services, while the membership fee supports enhanced access and services that insurance generally does not pay for.
APG is independently owned and physician led. Unlike a primary care office owned by a hospital system, APG is not built around keeping every referral inside one corporate network. Dr. Green can consider qualified specialists and facilities throughout the region and recommend the option he believes best fits your medical needs, preferences, insurance coverage, location, and timing.
Your APG concierge journey
Meet before you decide. Schedule a complimentary 15-minute conversation with Dr. Green to ask questions and determine whether the relationship is a good fit.
Choose your payment schedule. Pay $2,100 once annually, or select quarterly payments of $600 or monthly payments of $220.
Build the full picture. Begin with a comprehensive assessment of your history, current health, risks, medications, specialists, priorities, and goals.
Create your health strategy. Dr. Green will work with you to identify preventive priorities, needed follow-up, and practical next steps.
Use enhanced access throughout the year. Schedule longer appointments, receive same- or next-day access when medically appropriate and available, and contact Dr. Green directly after hours for urgent concerns.
Keep the plan current. Adjust the strategy as your health, medications, test results, and goals change.
Use the right resources. When you need care beyond APG, Dr. Green can consider qualified options across health systems rather than automatically directing you into one network.

More flexibility: A smaller, physician-led practice has fewer administrative layers and can communicate and make decisions quickly.
What happens when you need care quickly?
During normal business hours, you’d call us. After hours you can use the member contact method provided at enrollment. The team will determine the most appropriate next step, which may be a same-day or next-day office visit, telehealth consultation, medication guidance, testing, or emergency care. Direct access does not replace 911 or the emergency department for life-threatening symptoms.
What happens when you need a specialist?
Dr. Green can evaluate the need, explain why a referral is being made, and consider qualified options across hospital systems and independent groups. Clinical expertise, insurance participation, communication style, travel distance, urgency, cost, and appointment availability can all be part of the decision.
What concierge membership does not replace
Concierge membership is not health insurance and does not cover every medical expense. You should maintain Medicare or other health insurance for covered office services, specialists, hospital care, emergency care, prescriptions, imaging, and procedures. Copays, coinsurance, and deductibles may still apply.
