Before a referral

Dr. Green can:

• Share relevant history and clinical questions with the receiving office

• Help you prepare for the appointment

APG is not owned by a hospital system. That independence means the starting point is the specialist or facility that best fits you—not an automatic requirement to keep the referral inside one corporate network.

After a specialist visit

Recommendations from several clinicians can overlap or conflict. Dr. Green can help you understand:

• What the specialist found

• Which tests or treatments were recommended

• How a new medication fits with your existing medications

• What follow-up belongs with the specialist and what belongs with primary care

• Whether another opinion or option should be considered

• Which questions remain unanswered

If you are hospitalized

A hospital team focuses on the immediate episode. Your primary-care physician carries the longer story: your baseline health, medications, prior results, specialists, preferences, and goals.

Members should notify APG about emergency care or hospitalization as soon as practical. The team can help organize the information that returns to the practice and prepare for appropriate follow-up. Direct physician access does not replace 911, emergency services, or the hospital team.

After discharge

The days after hospitalization are when instructions, medications, appointments, and warning signs can become confusing. Follow-up may include:

• Reviewing the discharge plan

• Reconciling medication changes

• Clarifying which symptoms require urgent attention

• Coordinating recommended testing and appointments

• Reconnecting the episode to your ongoing health strategy
