Concierge Primary Care Frequently Asked Questions

What is concierge primary care?

It is a membership-based model that allows a physician to care for fewer patients and provide longer visits, enhanced access, closer follow-up, greater continuity, and more time for prevention and health planning.

Is APG a direct primary care practice?

APG Concierge Primary Care is a concierge model that works alongside Medicare or health insurance. Covered medical services may still be billed to insurance; the membership supports enhanced access and services insurance generally does not cover.

Do I still need health insurance?

Yes. Membership does not replace Medicare or health insurance. You should maintain coverage for specialists, hospital care, emergency care, prescriptions, imaging, procedures, and other covered services.

What does membership include?

Benefits include longer appointments, same- or next-day access when medically appropriate and available, direct after-hours physician access for urgent concerns, proactive health planning, closer follow-up, care coordination, and independent referral guidance.

Are visits unlimited?

Membership is designed to provide appropriate access based on medical need. Final visit policies should follow the signed membership agreement and APG's current operational guidelines.

How long are appointments?

Concierge visits are generally scheduled for 30 to 60 minutes, giving you time to discuss concerns and understand the plan. Traditional insurance-based appointments are typically limited to 15 minutes maximum.

Can I reach Dr. Green after hours?

Members receive a direct method for reaching Dr. Green about urgent after-hours concerns. This access does not replace 911 or emergency services for life-threatening symptoms.

Can I always get a same-day appointment?

APG offers same- or next-day access when medically appropriate and available. The team will help determine the safest next step based on your symptoms.

Is Dr. Green limited to one hospital system?

No. APG is an independent private physician practice and is not owned by a hospital system. Dr. Green can consider qualified specialists and facilities across the region based on clinical expertise, insurance, preferences, location, urgency, and availability.

What happens if I am hospitalized?

Notify APG as soon as practical. Dr. Green can help review information that returns to the practice, reconcile changes, and organize appropriate follow-up. The hospital team remains responsible for your inpatient care.

Does the membership fee cover testing and specialists?

Generally, no. Outside labs, imaging, prescriptions, specialists, emergency services, hospital care, and procedures are billed separately by the providers involved and may be covered by insurance according to your plan.

What are the payment choices?

  • $2,100 paid once annually

  • $600 quarterly—four payments, totaling $2,400 per year

  • $220 monthly—twelve payments, totaling $2,640 per year

The annual option has the lowest total cost.

Can I use HSA or FSA funds?

Tax rules and plan policies vary. Ask your plan administrator or tax adviser whether any part of the fee may be eligible. APG does not provide tax advice.

Can I cancel?

Yes. Cancellation, renewal, refund, and notice terms are governed by the membership agreement provided before enrollment.

Where does Dr. Green see patients?

APG Concierge Primary Care is located at 4414 West Center Drive in Alton, Illinois 62002.

How do I know whether it is right for me?

Schedule a complimentary 15-minute Meet & Greet. It is a no-pressure opportunity to ask Dr. Green questions and decide whether the relationship matches what you want from primary care.
