Dr. David B. Yablonsky

Dr. David B. Yablonsky serves as Chief Medical Director for Associated Physicians Group. He is also the Director of Internal Medicine.
Dr. Yablonsky is a board certified internist. Internists, sometimes referred to as “the doctors doctor”, are trained to diagnose and treat complex acute and chronic diseases in almost every adult organ system. They are also well versed in more simple ailments and in disease prevention. A native of Minneapolis, Minnesota, he earned his undergraduate degree from Boston University (Cum Laude). He attended Des Moines University College of Osteopathic Medicine. After earning his Doctor of Osteopathic Medicine, he completed a three year residency in Internal Medicine at the University of Rochester School of Medicine in Rochester, New York. Past leadership positions include Chief of Medicine, Internal Medicine Section Chief, Medical Director of Cardiopulmonary Rehabilitation and Chairman of Pharmacy and Therapeutics Committee at Grinnell Regional Medical Center in Grinnell Iowa.
Dr. Yablonsky has served thousands of patients as an internal medicine physician in Grinnell Iowa and the bi-state region. He was also a Clinic Physician at Southern Illinois University at Edwardsville. Dr. Yablonsky has owned and operated non- invasive cardiovascular diagnostic labs offering stress testing, echocardiography, peripheral vascular ultrasound and cardiac rhythm monitoring. He has extensive experience as a consultant to the medical legal industry as an expert witness and has testified at many trials though out the country. Dr. Yablonsky is an expert in the business and clinical aspects of Medicare Advantage and sits on the board of Southern Illinois Healthcare Association, the largest independent practice association in the metro east.
Dr. Yablonsky offers a full range of traditional outpatient medical services. His focus is on disease prevention and health optimization. He also specializes in diagnosing and managing a comprehensive array of adult acute and chronic disease including, cardiovascular diseases, diabetes, hypertension, lipid disorders, kidney disease, infections, cancers, pulmonary and respiratory diseases, gastrointestinal diseases, musculoskeletal diseases, mental illnesses, skin diseases, neurological diseases, obesity and male hormone optimization. Dr. Yablonsky oversees the medically supervised “Ideal Protein” weight loss method helping patients lose weight and improve their health. Dr Yablonsky is also currently the Director of Internal/Family Medicine at Associated Physicians Group.

Dr. Tony Chami, MD, DABA, DABPM
Board Certified Anesthesiologist and Pain Medicine Physician
Dr. A. Tony Chami is Board Certified in Anesthesiology and Pain Medicine. He is a Diplomate of the American Board of Pain Medicine, American Board of Anesthesiology, American Board of Medical Examiners, and American Academy of Pain Management.
Dr. Chami earned his undergraduate degree from Michigan State University in East Lansing, Michigan and Medical Degree from Wayne State University in Detroit, Michigan. He completed post-doctoral training in Anesthesiology at the University of Michigan Medical Center and University of Illinois Hospitals. He also completed post-doctoral training at Saint John Hospital and Medical Center. Dr. Chami has served as Medical Director at Chicagoland Advanced Pain Specialists, and Interventional Pain Specialist throughout the Chicago area and in Peoria, Illinois. He has co-authored literature to advance his specialty, and has presented multiple educational talks to community and professional groups. Dr. Chami has received advanced training certification in a broad array of pain management techniques; he works with a team of other specialties to achieve the best possible outcome to relieve his patients’ pain, and improve their quality of life .
Dr. Chami’s ’ primary areas of interest are spine-related pain conditions and the exciting new field of regenerative treatment to reverse degeneration in the spine and joints. He offers a wide range of pain management techniques such as image-guided epidural and selective nerve injections, facet joint treatment protocol including radiofrequency nerve ablation, disc diagnostic and therapeutic procedures, Dorsal column stimulator (pain pacemaker) implantation, complete knee and hip pain treatment protocol including radiofrequency ablation where appropriate, Kyphoplasty, and sympathetic blocks.

Dr. Chad Ermis, DO
Dr. Ermis earned his Bachelor of Science in Biology/Physiology from Northern Michigan University and his osteopathic degree from Kirksville College of Osteopathic Medicine, Kirksville, MO.
Dr. Ermis completed his OMS3/OMS4 at Genesys Regional Medical Center, Grand Blanc, MI. He completed his PM&R internship at William Beaumont Hospital, Royal Oak, MI.
His Physical Medicine and Rehabilitation residency was completed at William Beaumont Hospital, Royal Oak, MI. He then completed his Spine and Pain Management fellowship at Cleveland Clinic Hospital, Cleveland, OH.
Dr. Ermis’s addition to the practice will ensure greater accessibility and increased services to our patients. Dr. Ermis has broad clinical interests which include chronic/complex pain syndromes utilizing interventional modalities and multidisciplinary pain management. His clinical interest is interventional pain management procedures.

Dr. Blake Randles
Diagnostic Physician, Musculoskeletal Ultrasound Practitioner
Dr. Blake received his Bachelor’s degree in Radiologic Science with emphasis in diagnostic ultrasound from Southern Illinois University in Carbondale. He also completed the pre-medical accelerated science program at Scott Community College and went on to obtain his Doctor of Chiropractic from Palmer College of Chiropractic in Iowa.
He is a board certified, musculoskeletal diagnostic medical Sonographer with the American Registry of Diagnostic Medical Sonographers with ultrasound experience from multiple medical settings. Throughout his career, Dr. Blake has delivered abdominal, surgical, OB, gynecological, neonatal brain, vascular, and musculoskeletal ultrasounds.
He performs diagnostic ultrasounds in APG’s three clinics (Swansea, O’Fallon, & Edwardsville) and will provide the ultrasound guidance needed for certain injections done by the medical provider. He is certified in CPR, AED, and First Aid.

Dr. Jeff Davie
Dr. Jeff Davie did his undergraduate study at the University of Windsor in Ontario, Canada where he completed his prerequisite courses to enter chiropractic college. After completing his studies, he earned a Doctorate of Chiropractic at Logan College of Chiropractic in St. Louis, Missouri. Dr. Davie has also received additional training in Sports Medicine and is certified in acupuncture.
Dr. Davie specializes in the non-surgical treatment of back pain, neck pain, headache, leg and arm pain, peripheral joint problems, peripheral neuropathies, vertigo, dizziness and whiplash. As a member of Associated Physicians Group, he collaborates with the entire team of specialists to deliver comprehensive care focused on each patient’s total health and wellbeing.
He is a member of the Missouri Chiropractic Association and is licensed in Missouri and Illinois and primarily serves the APG offices located in Edwardsville and Swansea.

Dr. Amanda Kurtz
Dr. Amanda Kurtz is a board certified Chiropractic Physician. She completed her undergraduate studies with a Bachelor’s Degree in Human Biology from Logan College of Chiropractic. She then continued her education at Logan College of Chiropractic where she earned her Doctorate Degree in 2014. Dr. Kurtz also completed a residency as a Lead Intern within the Human Performance Center in Logan’s Montgomery Health Center. Throughout her residency program she treated patients within the VA hospital, St. Patrick’s clinic, and the Adult Rehabilitation Center in St. Louis, MO.
Dr. Kurtz has served as a guest speaker at the Professional Football Chiropractic Society seminar in Indianapolis, IN and has been a student advocate for The Foundation for Chiropractic Progress. Prior to joining the APG team Dr. Kurtz worked as a Professor at McKendree University where she taught Biology, Chemistry and Physics.
As a member of the APG staff Dr. Kurtz collaborates with all the treatment providers to create a well rounded and personalized treatment plan for each patient’s unique needs. Dr. Kurtz specializes in a variety of adjusting techniques and is able to adapt to each individual case. Dr. Kurtz has special interest in a wellness lifestyle to not only treat the underlying cause of pain but also to prevent any future recurrence.
Dr. Kurtz serves patients primarily at the O’Fallon location.
